Should I include bars, tone, countdown, or slate. Do you consider an opening title to be a "slate"?

Whatever you submit will be aired on TV as-is, in its exact form, via TV, so if you want the audience to see an opening title, please include it. Don’t put in anything extra for a Quality Controller, or Programming Director, since we have none. To be specific, no bars and tone, no countdown, no slate. You will provide the title as part of the ingest process, so all we require is the program itself.
Can I have a few sponsors advertised in my program ?
Answer
No, all advertising, sponsorship, and/or underwriting recognition must be coordinated through Deproduction/Denver Open Media. Deproduction/Denver Open Media has established an underwriting package that can help producers solicit underwriting in a fashion that is consistent with our policies. All underwriting recognition must follow all underwriting guidelines, and must be approved by Deproduction/Denver Open Media, as listed in the underwriting section of the website.

Denver Open Media stands for free speech and we will never censor information or opinions on our station. I've never even heard of the documentary you reference, but you're right that we support it, as we do all forms of expression and education. If you want to produce a show debating any information or perspectives presented in the film you mention, we'll gladly run yours side by side with "Gaza Strip".
